What are cookies, web beacons and similar technologies

Electronic Consent

Commit 2 Conquer cancer (Commit 2 Conquer) interested in engaging patients, their caregivers, and/or family members who would like to share their experiences with cancer through Talk Teal Talk Pink Cancer®, an outreach  initiative to increase awareness and provide information to people affected by cancer -- from prevention through survival.


If you are under the age of 18, you are not eligible to participate, and we ask you not to submit any personal information to Commit 2 Conquer.


For the purpose of advertising and/or promoting Talk Teal Talk Pink® initiative, I authorize Commit 2 Conquer, its affiliates, licensees and contractors that service Talk Teal Talk Pink® to use and/or disclose the health information I supply (“Health Information”). Commit 2 Conquer its affiliates, licensees and its contractors will not disclose the Health Information for any other purpose, except as required by law. I understand that once the Health Information has been shared with Commit 2 Conquer, federal privacy laws may no longer protect it from further disclosure.

My authorization will expire in 10 years, or less if required by state law, unless I cancel it sooner. I may cancel this authorization at any time and for any reason. To cancel this authorization, I agree to notify Commit 2 Conquer via privacy@commit2conquer.org. Once Commit 2 Conquer receives and processes my cancellation email, Commit 2 Conquer will not use the Health Information going forward. I understand that cancelling my authorization will not affect any use or disclosure of the Health Information by Commit 2 Conquer its affiliates, licensees, or its contractors that occurred before my request was received and processed.

On my own behalf, and on behalf of my heirs, executors, administrators, successors and assigns, I release and discharge the Released Parties from any liability by virtue of the exercise of their rights in the Publicity Rights, including without limitation any blurring, distortion, alteration, optical illusion, or use in composite form that may occur or be produced regarding the use of my name or the taking of said photograph(s), statements made by me, motion pictures, videotape, recordings, internet posting or other web publication, or other likeness of me in any processing thereof through completion of the finished product, and including without limitation any liability based on rights of publicity, invasion of privacy and other civil rights, moral rights, copyright infringement, defamation, libel and slander. I waive any right that I may have to inspect or approve any uses made of the Publicity Rights in connection with this consent/release.

Commit 2 Conquer Cancer Inc. ("Commit 2 Conquer") is committed to maintaining privacy protections for its users. We respect the privacy of every visitor to our website. Children Under the Age of 18

If you are under 18, do not use or provide any information about yourself to us, including your name, address, telephone number, email address, or any screen name or user name you may use. If we learn we have collected or received personal information from a child under 18 without verification of parental consent, we will delete that information. If you believe we might have any information from or about a child under 18, please contact us at compliance@commit2conquer.org.


I. INFORMATION WE COLLECT 

We collect “Non-Personal Information” and “Sensitive and Personal Information.” Non-Personal Information includes information that cannot be used to personally identify you, such as anonymous usage data, general demographic information we may collect, referring/exit pages and URLs, platform types, preferences you submit and preferences that are generated based on the data you submit and number of clicks. Sensitive Information is collected from users who are applying for assistance for transportation to medical treatments and for medical treatments; and during the application process, we may request information such as social security numbers, insurance policy numbers, pharmacy information, medical conditions, medications you use to determine which patient assistance programs can properly assist you. Your medical conditions may be considered Protected Health Information (“PHI”) and therefore sensitive personal information. You may elect not to provide your medical condition or other sensitive personal information, but you may not be able to participate in patient assistance programs through the Website. We do not use Cookies to enable ad targeting based on sensitive personal information.

Sensitive information is collected from donors and sponsors who make donations to the Foundation or sharing content in our social media accounts, please do not disclose sensitive personal information (information that can be used on its own or with other information to identify, contact, or locate a single person, or to identify an individual in context, such as protected health information, social security numbers, or healthcare account numbers) unless we specifically request your consent. 

Personal Information includes information that you may provide to us to register for a program or to sign up for our mailing list.  

To use the Service you do not need to submit Personal Information.  However, in an effort to improve the quality of the Service, we track information provided to us by your browser or by our software application when you view or use the Service, such as the website you came from (known as the “referring URL”), the type of browser you use, the device from which you connected to the Service, the time and date of access, and other information that does not personally identify you.  We track this information using cookies, or small text files which include an anonymous unique identifier.  Cookies are sent to a user’s browser from our servers and are stored on the user’s computer hard drive.  Sending a cookie to a user’s browser enables us to collect Non-Personal information about that user and keep a record of the user’s preferences when utilizing our services, both on an individual and aggregate basis.    

The Company may use both persistent and session cookies; persistent cookies remain on your computer after you close your session and until you delete them, while session cookies expire when you close your browser. 


 II. HOW WE USE AND SHARE INFORMATION 

Personal Information:  

Except as otherwise stated in this Privacy Policy, we do not sell, trade, rent or otherwise share for marketing purposes your Personal Information with third parties without your consent. We do share Personal Information with vendors who are performing services for Commit 2 Conquer, such as the servers for our email communications who are provided access to user’s email address for purposes of sending emails from us.  Those vendors use your Personal Information only at our direction and in accordance with our Privacy Policy.  

In general, the Personal Information you provide to us is used to help us communicate with you. For example, we use Personal Information to contact users in response to questions, solicit feedback from users, provide technical support, and inform users about promotional offers.  


Non-Personal Information 

In general, we use Non-Personal Information to help us improve the Service and customize the user experience.  We also aggregate Non-Personal Information in order to track trends and analyze use patterns on the Site.  This Privacy Policy does not limit in any way our use or disclosure of Non-Personal Information and we reserve the right to use and disclose such Non-Personal Information to our partners, advertisers and other third parties at our discretion.  

In the event we undergo a business transaction such as a merger, acquisition by another company, or sale of all or a portion of our assets, your Personal Information may be among the assets transferred.  You acknowledge and consent that such transfers may occur and are permitted by this Privacy Policy, and that any acquirer of our assets may continue to process your Personal Information as set forth in this Privacy Policy.  If our information practices change at any time in the future, we will post the policy changes to the Site so that you may opt out of the new information practices.  We suggest that you check the Site periodically if you are concerned about how your information is used.

Commit 2 Conquer is committed to respecting the privacy of its financial and in-kind donors, whether the donation is made online, by mail, or any other method. When you make a financial or in-kind donation, you provide your name and contact information. Financial donors provide standard payment information including, in many circumstances, credit card data. We protect your information in a variety of ways, as described below:


We use industry standard safeguards to protect your information. Many donors use credit cards for donations. You are responsible for the accuracy and legitimacy of any credit card information you provide on the Sites in order to make purchases or donations.  By providing the information, you represent that such information is accurate, complete and correct and that you have the lawful right to provide such information.  In order to make donations using the payment method displayed on the Sites, you must be 18 years of age or over. Billing to your credit card or to your account occurs at the time of donation or shortly thereafter. You expressly authorize the Company to charge the credit card provided by your or listed on your account for any donations submitted online via the Sites.  


The Company uses industry-standard SSL (secure socket layer) technology to protect your information and provide a safe and secure environment for online donations. It will not sell, trade or share your personal information with anyone else, nor send donor mailings on behalf of other organizations, and will only share personal information if you have given it specific written permission to do so. When we work with other companies to process credit card transactions, those companies also use encryption and other security measures.​  

​Board/Volunteers  

Board members are volunteers and, as such, have the same right to privacy as volunteers, which includes that any public information is limited to the name and corporate affiliation as may be necessary for compliance in non-profit governance.  Any other information must have expressed permission by the Board Member.

We implement security measures designed to protect your information from unauthorized access.  We protect your information from potential security breaches by implementing certain technological security measures including encryption, firewalls and secure socket layer technology.  However, these measures do not guarantee that your information will not be accessed, disclosed, altered or destroyed by breach of such firewalls and secure server software.  By using our Service, you acknowledge that you understand and agree to assume these risks.

Applying to work or volunteer at Commit 2 Conquer: When you apply to be a Commit 2 Conquer employee or volunteer, we ask for a variety of types of data that are typical for employment applications, and we may ask for your résumé.  

Making a financial contribution: When you make a financial contribution online, we collect your name, contact information and standard credit card data. If you provide Commit 2 Conquer with personal information when you visit our website, we may enter your name into one of our databases so we can contact you to obtain your input, provide information about our programs and events, or request contributions.​

Return Policy 

​Your complete satisfaction is important to us. If you are not satisfied with your purchase, let us know. We have a 30 Day Return Policy for items in original condition, excluding personalized or customized items.  

 

30 Day Return Policy

Commit 2 Conquer Cancer is proud of the product lines we have created, and we are committed to your complete satisfaction.  We are happy to accept returns on items that are in the original condition and packaging. All products must be returned to us within 30 days of the date of purchase. 

Stock items may be returned within 30 days provided the items have not been used or washed.   We do not offer exchanges. If you would like a different item, feel free to place a new order on our web site.

Any items that have been customized or personalized can not be returned unless they are defective. Freight will be credited to your account for defective products only.


Returning a Defective or Damaged Item  

Sometimes, in spite of our best efforts, a defective, damaged or incorrect item gets sent out.  When this happens, please contact us immediately via email at shop@commit2conquer.org or via phone at 678-551-4587. To expedite the process, we ask for your order number as well as the details of the problem.  We will be happy to send a pre-paid shipping label for you to use to return the item to us for a replacement or a full refund. 

  

All returned items will be inspected by our staff and a credit issued within 10 days of receipt of the items. If an item is deemed non-returnable we will make contact directly with you.
